Several philosophers have recently argued that phenomenology is well-suited to help understand the concepts of health, disease, and illness. The general claim is that by better analysing how illness appears to or is experienced by ill individuals—incorporating the first-person perspective—some limitations of what is seen as the currently dominant third-person or ‘naturalistic’ approaches to understand health and disease can be overcome. In this article, after discussing some of the main insights and benefits of the phenomenological approach, I develop three general (...) critiques of it. First, I show that what is often referred to as naturalism tends to be misunderstood and/or misrepresented, resulting in straw-man arguments. Second, the concept of normality is often problematically employed such that some aspects of naturalism are actually presupposed by many phenomenologists of medicine. Third, several of the key phenomenological insights and concepts, e.g. having vs. being a body, the alienation of illness, the epistemic role of the first-person perspective, and the idea of health within illness, each bring with them new problems that limit their utility. While acknowledging the possible contributions of phenomenology, these criticisms point to some severe limitations of bringing phenomenological insights to bear on the problems facing philosophy of medicine that should be addressed if phenomenology is to add anything substantially new to its debates. Various scientists and philosophers have heavily criticized what they see as problematic forms of ‘nutritional reductionism’ or ‘nutritionism’ whereby studying food–health interactions at the level of isolated food components produces largely misguided science and misleading interpretations. However, the exact target of these diverse criticisms remains elusive, and its implications are overstated, which may hinder scientific understanding. To better identify the types of flaws supposedly hindering reductionist research, we disentangle three types of reductionist claims to better determine what the debate is (...) about and to propose ways to move it forward. We then present a qualified defence of the reductionist programme that hinges on the ability to identify nutritional causes that make a difference, which we illustrate through central examples in the history of nutrition. This defence is taken further by using insights from the philosophy of mechanisms to analyse how the field of nutritional ecology offers a synthetic framework to explain, generate, and test predictions about nutrient–organism interactions, which is premised on the biological mechanisms of nutrient-specific appetite regulation. The result, which we call ‘synthetic reductionism’, avoids many, though perhaps not all, of the challenges raised by anti-reductionists, and also highlights the potential of reductionism to identify nutritional difference makers. One of the key criticisms of understanding health in terms of adaptation to one’s environment is that medical judgments should be able to apply across environments. If we say that a condition is pathological ‘for person X in environment E’, then we quickly run into problems of desirability and social values. However, many key concepts in biology entail an inability to separate the organism from its environment. In other words, it is precisely by referring to ‘organism X in environment E’ (...) that we can determine what is ‘normal and natural’. In this chapter, I will argue that the role of this inseparability of organism and environment for understanding medical norms has been misunderstood and the implications of it for naturalistic theories of health and disease have gone largely unappreciated. To better understand this contextualist approach, I will discuss the ideas of John Ryle and Georges Canguilhem, focusing primarily on the latter. In Canguilhem’s work we find some key arguments for why organismic norms need to be understood relative to environments and how this can help to clarify the concepts of health and disease. I will explore his peculiar form of naturalism that was based on the dynamism and variability of organisms, show how it can be clarified through more recent biological research, and mention some of its limitations. This essay discusses four key criticisms recently leveled against the main attempts to use conceptual analysis to understand health and disease. First, it examines the weaknesses of these attempts and suggests a better way to proceed. Next, it briefly discusses another disease debate concerning pathological mechanisms and suggests that this approach could be more fruitful than that of conceptual analysis. The final section demonstrates how Georges Canguilhem's biological philosophy of disease avoids some of the problems associated with conceptual analysis, and (...) how he can contribute to both debates. Canguilhem's appeal to biological normativity is understood as contributing to a theoretical explanation, and a possible naturalization, of what is specific to healthy and pathological phenomena. This article analyses the role of sacred and religious art in Amazonia in the context of the catholic renovation movement of the 19th century, known as Catholic Reform. For that, it takes the order of the main altar of Our Lady of Belém by Pará’s Bishop Antônio de Macedo Costa to Italian architect Luca Carimini as its study object. This piece, along with others made throughout the period of Pio IX’s pontificate, constitutes an iconological programme as well as an artistic (...) and sacred narrative body, which testify to the pedagogical and political debates on the history of Roman Catholicism in Brazil in the 19th century. Besides that, it moves artists that acted under the patronage of Pope Pius IX in Latin American cities such as Santiago, in Chile, and Belém do Pará, in Brazil, putting in evidence the artistic, intellectual and religious connections between the Vatican and South America, as part of an international renewal movement of Catholicism. This paper presents an integrated framework applied towards the design and evaluation of a virtual museum of Byzantine art that combines the theorized fields of semiotics, virtual heritage (VH), and Byzantine art. A devised semiotic model, the case study semiosphere, synthesizes important principles from the theoretical background justifying the overall design and evaluation methodology. The approach presented has theoretical extensions to the understanding of the role technology plays in promoting a consummatory aesthetic experience for Byzantine art in virtual environments, complementing (...) the experience received from traditional Byzantine art media. Part A of the work presents the development of the semiotic foundation of the study prior to presenting the applied potential of the approach in design and evaluation of VH for Byzantine art, which appears in Part B. The final task of the proposed approach aims to support a meaningful interpretation, assisting in the promotion of the significance (value) of the virtual museum to potential interpreters/visitors. Continuing from Part A, in which we discuss the semiotic foundation for designing a virtual museum of Byzantine art, Part B presents an applied methodology for the representation of cultural artifacts through virtual technologies and semiotic techniques. We discuss how our semiotic model, case study semiosphere, contributes to design and evaluation research of such unique art-form representation and why the approach contributes as a whole to the field of Virtual Heritage. Medicalization appears to be an issue that is both ubiquitous and unquestionably problematic as it seems to signal at once a social and existential threat. This perception of medicalization, however, is nothing new. Since the first main writings in the 1960s and 1970s, it has consistently been used to describe inappropriate or abusive instances of medical authority. Yet, while this standard approach claims that medicalization is a growing problem, it assumes that there is simply one “medical model” and that the (...) expanding realm of “the medical” can be more or less clearly delineated. Moreover, while intended to establish the reality of this growing threat, this research often requires making arbitrary or unjustified distinctions between different practices. To better clarify the concept of medicalization, I will focus more on capturing the variety of medical practices than on the sociological aspects of medical discourse. In doing so, I will explore the distinction between medicalization and pathologization, a distinction that is often overlooked and that brings with it many conceptual and practical implications. After defining these terms, I will use some examples to show that while pathologizing is closely tied to medicalizing, both can occur independently. I will then further develop this distinction in terms of the different individual and social effects of these practices. (shrink)

The idea that the function of language is its use is commonly ascribed to the Later Wittgenstein. In this paper, I argue that there is textual evidence already coming from the early manuscripts proving that Wittgenstein's philosophical development is culminating in the idea of function as use around 1929–30. I interpret a passage from Ms‐107 in order to show that Wittgenstein's practical turn has sources in different stages of his philosophical development, each of which is dominated by different ideas: the (...) idea of a picture theory; the idea of a phenomenological language and the idea of function as use. While aging research and policy aim to promote ‘health’ at all ages, there remains no convincing explanation of what this ‘health’ is. In this paper, I investigate whether we can find, implicit within the sciences of aging, a way to know what health is and how to measure it, i.e. a theory of health. To answer this, I start from scientific descriptions of aging and its modulators and then try to develop some generalizations about ‘health’ implicit within this research. After (...) discussing some of the core aspects of aging and the ways in which certain models describe spatial and temporal features specific to both aging and healthy phenotypes, I then extract, explicate, and evaluate one potential construct of health in these models. This suggests a theory of health based on the landscape of optimized phenotypic trajectories. I conclude by considering why it matters for more candidate theories to be proposed and evaluated by philosophers and scientists alike. (shrink)

This article explores Herodian's History of the Roman Empire alongside Chariton's novel Callirhoe with an eye to how the minds of collective entities are represented and function in the two narratives. It argues that Chariton, unlike Herodian, elaborates on the diversity of emotions that characterizes a specific collective experience and has groups use direct speech throughout. These choices add vividness to the narrative and intensify the fictional sensationalism and dramatic character of the novel. It also shows that, whereas collectives in (...) Chariton's narrative are primarily designed to highlight a specific characteristic of a hero, dramatize an event and enhance suspense, in Herodian's historiography they are an integral part of the plot and central to his historical analysis of contemporary political and social world. This article offers a new analytical tool geared towards the development of a poetics of the collective in ancient narrative as well as a poetics of fictional and factual narration in antiquity, and advances our understanding of the complex relationship between ancient historiography and novelistic writing. In this introduction we present Patrick Duffley's book Linguistic Meaning meets Linguistic Form, as well as the contributions that each scholar has brought into the debate on linguistic meaning and form. They deal with semantic and foundational issues regarding a sign-based approach to meaning.

In this paper we suggest that Duffley’s sign-based semantics rests on two main claims: a methodological one and an ontological one. The methodological one is the analysis of corpora and the ontological one is the postulate of mental content. By adopting a linguistic enactivist perspective with a Wittgensteinian twist, we endorse Duffley’s methodological claim and suggest that a sign-based semantics doesn’t have to rely on mental content if it takes into account the conception of meaningful material engagement in cognitive archeology (...) and its development into sign-using as an enactive capacity. Phronesis is essential for good decision‐making and actions. This literature review shows how phronesis has been discussed and related to elements of the field of administration and organizations. A search in the database systems Scopus, EBSCO, Web of Science, and Scielo, based on eligibility criteria, resulted in 43 theoretical and 14 empirical works. The analysis of these studies showed the most significant empirical contributions, the most cited authors, methods, journals, and central themes addressed in studies on phronesis to understand ethics (...) in business. From a virtue ethics perspective, we discuss the major theoretical and empirical contributions, as well as the implications of a restricted understanding of phronesis when it is treated apart from other elements of the virtue ethics framework. We present a synthesis of the concepts of phronesis, based on dimensions of magnitude and amplitude of the conceptions, ranging from formal to substantive phronesis, and covering individual, organizational, or societal levels of analysis. We argue that a substantive conception of phronesis within virtue ethics may improve our understanding of ethics because it assumes the interdependence of phronesis and moral virtues, integral anthropology, and the common good. We present a future research agenda, considering the study's limitations and findings. (shrink)

This article offers a usage study of the word φήμη throughout Herodian’s Roman History. It sets Herodian’s practice in a broader literary picture that includes other historians, but also epic and the novel, and then suggests that the extremely high frequency of φήμη in Herodian is unique among Greek-language historians and that Herodian is indebted to Latin-language historiography for this technique. The following sections examine how Herodian perceives the phenomenon of φήμη and makes it a salient feature of his historiographical (...) strategy. The discussion shows that φήμη, regardless of its truth status, has multiple functions in Herodian’s narrative. First, it contributes to the portrayal of historical agents. Second, it is a crucial dramatic factor that signals turning points in the story and generates vividness and an atmospheric narrative. Third, it acts as a plot-driver in the story. Fourth, it has a role in the composition of the narrative by creating intratextual associations and narrative patterns. Finally, it has metanarrative ramifications, since there is a parallel between the use of φήμη by the historical agents and by Herodian himself. Overall, the analysis advances our understanding of Herodian’s narrative technique and his construction of historical truth. (shrink)

Defining the concepts of health and disease has proved rather difficult and many philosophers of medicine have simply concluded that we would be better off giving up on such endeavors. I feel that this view is misguided mainly because it seems to rest on a rather inadequate understanding of how philosophers use biology to clarify medical concepts. While some philosophers appeal to biology so as to clarify what we mean by the concepts of health and disease, others attempt to use (...) biology to develop a theory thathelps to explain what health and disease are. In this dissertation, I examine the work of an often overlooked philosopher of medicine, Georges Canguilhem, who sought to understand medical concepts by starting from the biological properties of variation and variability. In other words, in order to define health and disease, Canguilhem first tried to establish how what is normal can vary between organisms and even within the same organism due to the dynamic relation between the organism and its environment. Simply put: what is normal for one organism could be pathological for another and what is normal in one environment could become pathological in another. Consequently, variation and variability need to be included in our understanding of health and disease. Throughout this dissertation, I explore how Canguilhem goes about developing the implications of these biological properties for medical concepts and this allows me to distinguish his approach from some popular ones in philosophy of medicine, e.g. those trying to develop an objective biological approach (naturalism) and those appealingto evolutionary principles. I establish the plausibility of definingmedical concepts relative to individual organisms and their environments, while showing some of its potential limitations, especially when it comes to human social environments. Ultimately, a more nuanced understanding of how the inseparability of organisms and environments canhelp to clarify health and disease provides an interesting way to get past the apparent stalemate in philosophy of medicine and firmly establishes the relevance of Canguilhem’s philosophy. The unexpected roles of the microbiota in cancer challenge explanations of carcinogenesis that focus on tumor-intrinsic properties. Most tumors contain bacteria and viruses, and the host’s proximal and distal microbiota influence both cancer incidence and therapeutic responsiveness. Continuing the history of cancer–microbe research, these findings raise a key question: to what extent is the microbiota relevant for clinical oncology? We approach this by critically evaluating three issues: how the microbiota provides a predictive biomarker of cancer growth and therapeutic responsiveness, the (...) microbiota’s causal role(s) in cancer development, and how therapeutic manipulations of the microbiota improve patient outcomes in cancer. Clarifying the conceptual and empirical aspects of the cancer-associated microbiota can orient future research and guide its implementation in clinical oncology. (shrink)

While philosophers of science have marginally discussed concepts such as ‘nutrient’, ‘naturalness’, ‘food’, or the ‘molecularization’ of nutrition, they have yet to seriously engage with the nutrition sciences. In this paper, I offer one way to begin this engagement by investigating conceptual challenges facing the burgeoning field of nutritional ecology and the question of how organisms construct a ‘balanced’ diet. To provide clarity, I propose the distinction between nutrient balance as a property of foods or dietary patterns and nutrient balancing (...) as an evolved capacity to regulate nutrient intake. This distinction raises conceptual and empirical issues, such as what properties constitute this capacity and whether they are the same in all organisms. Additionally, while scientists use the term ‘balancing’, its intension and extension need further clarification. Based on the literature, the properties of external nutrient detection, internal sensing of nutrient levels, and organismal regulation could provide a basic recipe for nutrient balancing. Next, using an evolutionary lens, I examine nutrient acquisition in some prokaryotes, slime molds, simple multicellular eukaryotes, and in the quirks of multicellular metabolism to raise questions about the origins and universality of balancing. Finally, I build on this explication of balance and balancing by considering how obesity and cancer might respectively elucidate problems of organismal nutrient imbalances versus disrupted cellular nutrient balancing. We situate the well-trodden debate about defining health and disease within the project of a metaphysics of science and its aim to work with and contribute to science. We make use of Guay and Pradeu’s ‘metaphysical box’ to reframe this debate, showing what is at stake in recent attempts to move beyond it, revealing unforeseen points of agreement and disagreement among new and old positions, and producing new questions that may lead to progress. BackgroundOrgan transplantation represents the most effective and acceptable therapy for end-stage organ failure. However, its frequent practice often leads to a shortage of organs worldwide. To solve this dilemma, some countries, such as Portugal, have switched from an opt-in to an opt-out system, which has raised concerns about respect for individual autonomy. We aimed to evaluate whether young university students are aware of this opt-out system so that they can make informed, autonomous and conscious decisions, as well as to identify (...) the factors that determine a positive attitude toward post-mortem organ donation.MethodsAn observational, cross-sectional study was developed and a questionnaire was administered to first-year students from six faculties of the University of Porto.ResultsOf the 841 participants, 60% were unaware that Portugal had adopted an opt-out system. Among the informed individuals, their main sources of information included social media, internet, and family. Furthermore, only 48% of all participants agreed with the current opt-out system. Female sex (p = 0.049; OR 1.393), knowledge of the law (p < 0.001; OR 4.749) and family being the primary source of information (p < 0.001; OR 2.855) were independent factors associated with a positive attitude toward post-mortem organ donation law.ConclusionsThere is a significant lack of knowledge among young university students regarding the presumed post-mortem organ donation law and how it works. Female sex, having family as a primary source of information and being aware of the presumed post-mortem organ donation law are the strongest independent factors that determine a positive attitude toward the opt-out system. Debates in fields studying the biological aspects of aging and longevity, such as biogerontology, are often split between ‘anti-aging’ approaches aimed largely at treating diseases and those focusing more on maintaining, promoting, and even enhancing health. However, it is far from clear what this ‘health’ is that would be maintained, promoted, or enhanced. Interestingly, what few have yet to fully reflect on is that there is still no theory of health within the health or aging sciences that would provide an (...) integrative explanatory framework akin to other scientific theories. After clarifying why such a theory could be useful, I discuss five general features of medical theories that could be used to evaluate the utility of a given proposal. With these features in hand, I suggest that philosophers and scientists work together on analyzing actual medical research (experimental analysis), and the ways in which a theoretical construct of ‘health’ is being progressively identified and measured therein. I conclude by suggesting that research fields studying stress and aging might be particularly helpful in developing candidates for theory construction due to their broad scope, increasing specificity, and potential for providing integrative explanations. The word ‘persuasion’ can be used in two different ways. It can either implicate a process of changing someone’s belief or action without any argumentative justification or, to the contrary, indicate that the changes are indeed a result of argumentative discourse. These two different uses are part of a conceptual development in the history of philosophy. Nowadays they are often placed in contrast to each other, whereby persuasion in philosophy as a non-argumentative act is taken as doubtful and even unsound. (...) In the following paper I argue that the two types of processes should not be conceived of as incompatible, taking some notes from Wittgenstein’s On Certainty as an argumentative source. I will show that persuading as a rational practice is based on the possibility of persuasion as a process of inculcation. These processes can help illuminate the notions of good grounds, reasons and persuasion. In contrast to the common perspective, I will show that they are actually the normal case regarding interaction between humans and thus fundamentally provide the possibility of an argumentative practice of giving and accepting reasons. I conclude that the primacy of argumentative persuasion in relation to inculcation needs to be reversed and the common understanding of human beings as genuine rational acting beings needs to be questioned. In this concluding chapter, we gather the various contributions of this volume and attempt to extract some of the many key insights and challenges raised when it comes to the project of explaining health across the sciences. These insights were distilled down into a selection of the central concepts and issues defended or discussed by the authors, and were organized into a table to see, at a glance, where the attention was given. Reflecting on these insights will go some way (...) towards addressing the question of whether these explanations can be brought together into a coherent or unifying framework or whether we are simply stuck with mutually exclusive views about the multifaceted phenomenon of ‘health’. Understanding the phenomenon of health is crucial for ageing research since there is often an implicit view on what constitutes health and how to measure it. We provide some reflections on how we might better understand and measure health, discuss the basic biological principles of survival, ageing, age-related diseases and eventual death, and end by tying these ideas together to rethink the nature of and implications for healthy ageing. We defend a more positive view on health understood in terms of (...) various phenotypic parameters, such as robustness and resilience, and show how it relates to the aim to support these parameters over time to achieve healthy ageing. Together, these ideas suggest that the most effective strategies for healthy ageing may come from the outside-in by altering those aspects of the environment that prevent robust and resilient phenotypic responses and by supporting those aspects which ensure such responses. Logical Holism and Wittgenstein’s ‘Practical Turn’. – Logical holism is the idea that each elementary proposition belongs to a system and is logically connected to other propositions of that system. In this paper I explore this idea and draw its connections to the nature of negative propositions and the ‘problem of recognition’ on the basis of Wittgenstein’s Nachlass. In the first section I argue that in January 1930 the idea leads Wittgenstein to a better understanding of how the negative feature (...) is expressed in propositions, thereby raising the problem of recognition to which he is not yet able to find a proper solution. In the second section I explore how the problem still persists during Wittgenstein’s ‘practical turn’. What he now calls the ‘problem of representation’ forces him to change again his conception of propositions. In the final section I argue that this change is mainly due to the modification of his conception of hypotheses which urges a solution to the problem of representation, though the problem remains unsolved in January 1930. Cependant, on peut dégager l'originalité du politique chez Kant précisément à partir de cette difficulté d'accommodation. On verra alors s'esquisser un rapprochement stylistique entre Kant et l'idéalisme allemand: dans les deux cas, l'intelligibilité du présent est rapportée à la médiation entre normativité et (...) expérience. It seems that there is no place to politics in the philosophical doctrine orchestrated by the Copernican revolution. Indeed, none of the two parts of the philosophical system — nature and freedom — can alone give a satisfactory account of politics. However, it is arguable that by reflecting on this topical difficulty one could detect the originality of Kant's approach to the political. By proceeding in this way one is also able to recognize a stylistic nearness between Kant and the philosophers of German Idealism: in both cases, the intelligibility of the present is related to the mediation between normativity and experience. Neste texto, procuro encontrar as origens de um dos mais importantes conceitos de Gilles Deleuze, o conceito de Imagem-tempo. Este conceito remete-nos para os primeiros textos de Deleuze dedicados à filosofia de Espinosa e ao problema do autómato espiritual e relaciona-se directamente com o problema da passividade/actividade do espectador. Ou seja, o conceito crucial na sua filosofia do cinema, a Imagem-tempo, esconde uma importante reflexão sobre a Imagem cinematográfica como arte de massas, os (im)poderes do pensamento e o modo fascista (...) de se pensar. This text seeks to find the origins of one of the most important of Gilles Deleuze's concepts, the concept of Time-image. This concept leads us to his first texts regarding Spinoza's philosophy and the problem of the spiritual automaton, and concerns directly with the problem of passivity/activity of the film goer. That is to say that the crucial concept of his film philosophy, Time-image, hides a fundamental consideration on the cinematic image as a mass art, the (un)powers of thinking and the fascist way of thought. This essay explores the relation between repetition and thought in Bergson and Deleuze. In Bergson, this relation is seen in the method of intuition by which thought is made to think in time and in the ‘rhythms’ at work in how intuition is a contact with time or life, urging conceptual precision. This framework is used to clarify Deleuze's thought without image as that contingent encounter with the persistent forces of life that demand the perseverance of thought. Our objective in this paper is twofold: first, we intend to address the tenability of the enactivist middle way between realism and idealism, as it is proposed in The Embodied Mind. We do so by taking the enactivist conception of bringing forth a world literally in three conceptual levels: enaction, niche construction and social construction. Based on this proposal, we claim that enactivism is compatible with the idea of an independent reality without committing to the claim that organisms have cognitive (...) access to a world composed of properties specified prior to any cognitive activity. Our second goal is to show that our literal interpretation of bringing forth a world not only sustains the legitimacy of the middle way, but it also allows us to revive the conception of evolution as natural drift—which is perhaps the least examined aspect of the original enactivist theory and is central to the understanding of cognition in an enactive way. Natural drift focuses on how structural couplings between organism and environment trigger viable pathways of maintenance and reproduction, instead of selecting the most adapted trait to a pregiven environment. Thus, although enactivists typically do not explore the consequences of their views regarding evolutionary dynamics, we show how natural drift provides a suitable starting point. (shrink)

Philosophers of medicine have long debated the possibility of a/the definition of health, but they have yet to fully reflect on the intriguing observation that there is still no theory of health within the medical sciences similar to general theories in other sciences. In this chapter, I provide some reasons for why this lack persists and why philosophers have not been particularly helpful or even interested in filling it. After clarifying why such a theory could be useful, I discuss five (...) general features of medical theories and how one could evaluate the utility of a given proposal. With these features in mind, I suggest that philosophers and scientists work together on analyzing actual medical research (experimental analysis), and the ways in which a theoretical construct of ‘health’ is being progressively identified and measured therein. I conclude by suggesting that research fields studying stress and aging might be particularly helpful in developing candidates for theory construction due to their broad scope, specificity, and potentially integrative explanations.
